You will need to provide the last two months statements for all bank, building society and Post Office accounts that you hold.
We must receive these within 14 days, without this information we cannot consider your request. We will not write and tell you this.
If you do not provide your statements within 14 days your council tax account will not be reviewed. You will be expected to continue your payment instalments as billed.
If the account falls into arrears you will receive reminders requesting it be brought back up to date before any recovery action commences.
You can upload your evidence or send it in by post, further details will be included in your confirmation email.
The information you have provided on this form will be used to deal your request by the relevant department. We may pass your details on to contracted third party service providers. We will store a copy of your completed request as a backup for 12 months.
