Report if there has been a permanent or temporary change to the number of people in your household (you can also use this if your household income has changed as a result of someone moving in or out)

If you have used the Tell Us Once service when registering the death you do not need to use this form.

You will need to complete a separate form for each person who has joined the household.

You must provide a birth certificate. You can upload a copy on this form.

You must provide a copy of your new tenancy agreement showing your new rent liability.

If a person under 16 years of age is staying with someone other than a relative for 4 weeks or more, they must legally inform Children's Services tel no 208100. This includes families hosting Foreign Language students

As the person moving in is a joint tenant but not your partner, they will need to make a separate application for Housing Benefit.

You will need to supply evidence of their new earnings.

If you are reporting a birth a birth certificate is only required if a 'Tell Us Once' notification has NOT been received. We no longer need to ask for Tax Credit notifications.

You will need to provide proof of the income and capital of anyone who has moved into your property. Please send this to Torbay Council, Town Hall, Torquay TQ1 3DR or use the option at the end of this form to upload any documents you have showing their income or capital. Alternatively to provide proof of income, you can print Certificate of Earnings form and ask their employer to complete it.

Anyone who is eligible to vote is required by required by law to register for the Electoral Roll. You can print out a form using the following link Electoral Registration. Please send the completed form to Torbay Council, Town Hall, Torquay, TQ1 3DR. Alternatively, the Electoral Registration team will send you a form to complete. You must complete a form to be added to the Electoral Register.

Anyone moving over the age of 16 should complete a Change of address form.

We cannot amend your claim until we have received all supporting documents. Your claim will then be assessed as quickly as possible - please refer to the following web page for current processing times .

You will need to complete a separate form for each person who has left the household. Separate forms are not required for dependant children.

As the applicant has moved out the current resident will need to apply for Benefit in their name. A link to apply is given after submitting this form.

If you are the applicant and moving please complete a Change of address form to notify us of your new address

You will need to provide evidence of your new income ie Tax Credits, JSA.

If you have dependant children, they may be entitled to Free school meals.

Please note, the maximum total size you can upload is 25Mb.

Universal Credit

Torbay is now a ‘full service’ Universal Credit area. This means that if you receive Housing Benefit (HB) it will stop if you:

  • New job or number of hours worked ends Jobseeekers Allowance (JSA) Income Support (IS) or Employment Support Allowance (ESA IR) and child tax credits is not in payment
  • Receiving Working Tax Credits (WTC) and work finishes or is reduced to less than 16 hours per week
  • Receiving IS or JSA and you become unfit to work or look for work
  • Receiving Employment Support Allowance (ESA) and you are now able to work or look for work
  • Couples separating where Tax Credits is in payment as a couple
  • Receiving Income Support and youngest child reaches age 5 or leaves the household
  • Become responsible for a first child in the household

As you have reported one of the above changes your Housing Benefit will stop and you will need to make a claim for Universal Credit.

We still need you to submit this form so that we can take these changes into account for your exisitng claim to Housing Benefit/Council Tax Support.

After this form has been submitted we will provide further information about how to make a claim Universal Credit.

Data Protection

The information you have provided on this form will be used to deal with your request by the relevant department. We may pass your details on to contracted third party service providers. We will store a copy of your completed request as a backup for 12 months.

Changes you must tell us about- tell us straight away if:
  • any of your children become 18 years old
  • anyone moves into or out of your home (including lodgers and subtenants);
  • you move; or
  • anything you have told us about changes

If you phone to advise us of any changes, you may be asked to supply further details in writing.

You must make sure that you tell us about these changes. Don't rely on someone else to pass the message on.

Failure to provide this information within 21 days could lead to the issuing of a Penalty Notice for £70.00.
